Elephantiasis Nostras Verrucosa: A Case Report

Elephantiasis nostras verrucosa is a rare, chronic, progressive, deformative disorder characterized by nonfilarial lymphedema. Edema, jeneralized thickening, lichenification, cobblestone-like apperance and verrucous changes are seen in the affected area. 59-year-old man came to our department with the complaints of swelling in the left foot and leg, deformation, bad smelling, discharge and pain. It is learned that he is homeless, lives in the streets, has had his complaints nearly for 5 years and has not been treated before. On the dermatological examination there are nonpitting edema on the dorsum of the left foot, multiple verrucous nodules and papules, jeneralized thickening of the dorsal aspect of the toes, cobblestone-like skin, hyperkeratotic papules on the left pretibial area and between these papules hyperpigmented sclerotic lesions, varicous lesions on both feet, 2 cm diameter, purulent discharcing ulcers on the medial malleol of the left foot and lateral malleol of the right foot, subungual keratosis and discoloration on the first nail of the right foot. The systemic examination of the patient is normal. WBC: 5500/mm³, Hb: 11.1g/dl, sedimentation: 60 mm/h, CRP: +++ have been found in the laboratory findings, liver and kidney function tests are normal. Metisiline sensitive staphylococcus coagulase (-) and nonfermantative gram (-) bacillus have grown in the culture of the left foot ulcer. There is no growing on the fungus culture of the left foot. Antibiotic treatment has been applied with the suggestion of infection department. Orthopedics and plastic surgery departments have informed that they can make a surgical operation if osteomyelitis is extracted. But the patient has left the hospital without informing us, so treatment has not contiuned.
