The Value And Efficacy Of Histopathologic Criteria For The Diagnosis Of Early Stage Mycosis Fungoides

Objectıve: Our aim to determine the frequency of histopathologic features in the biopsies diagnosed as mycosis fungoides ( MF) early stage and to evaluate the value of diagnostic criteria of MF early stage claimed to be reliable by several published data.
Materıal and Methods: One hundred twenty two skin biopsies which were clinicaly suspected of MF and pathologically evaluated and reported as consistent with MF early stage were included in this study. Totally 90 skin biopsies which were histopathologic simulators of MF early stage and/or clinically suspicious for MF were also included in this study. Thirteen different histopathologic parameters classified as epidermal changes, dermal changes and lymphocytic atipia were reviewed in total 212 skin biopsies.
Results: Histopathologic evaluation of 13 different parameters by employing multivariate logistic analysis has shown the presence basal alignment of lymphocytes, the fact that epidermal lymphocytes are larger than dermal lymphocytes and accompanying disproportinate epidermotropism are independent diagnostic factors. Those factors also have more differential diagnostic powers. On the other hand, due to their specifity of 100%, differential diagnostic power of Pautrier microabscesses and prominent lymphocytic atypia has been established albeit the rare occurence of these at MF early stage.
Conclusion: We thought that, the presence of basal alignment of lymphocytes, epidermal lymphocytes are larger than dermal lymphocytes and disproportinate epidermotropism were realiable histopathologic features for diagnosis of MF early stage when the clinicopathologic correlation was provided.
